They usually don't tear completely, they start out as a partial tear and get worse. Its a catch 22, the best way to prevent a full tear is to strengthen the muscle around it, best way to fully tear it is to overwork the muscle around it were it can no longer hold it together. its usually a problem all your life, best thing to do is to be aware of it, especially periods of going back to the gym after hiatuses and any type of heavy lifting when your out of shape. Same goes for out of shape weekend warrior football games, batting cages etc.
I got mine bouncing off a bmw that made a left turn in front of me. I had to immobilize it for a while. luckily 6 years later, knock on wood, no surgery. I do get a painful reminder time to time.
