most likely there was a hairline flaw in the metal the reg was made of and the reg split along that line.

it could also have been an impurity in the metal the reg was made of that caused the failure.

if this is happening to a large number of the same Reg's, i would suspect that there is a bad production run.
if it is only happening when installed to a specific bottle, i would first check into who is doing the install.
it could be that by over torquing the regs they are cracking the metal and creating a missile waiting to be launched.

when storing your co2 or hpa bottles i recommend that you put them in some kind of container like a tool box or cooler.
the cooler (with no ice) will keep the temperature stable for the co2 and the container will remove energy or deflect any flying objects in the event that for some reason the bottle fails.
it's a simple easy thing to do, and it will reduce stories like this.

we don't need the small minds with big mouths that want to ban everything fun to have any more reason to beef about Paintball.
