check to see if the allen screw in the bolt is stripped out and when it goes forward, the screw flies forward plugging the hole. Also check that brass valve stem guide in the valve. Make sure it is not backing its way out and it is tight. also check the hammer oring to make sure its not a chris mader hammer oring or a tank oring or that its mushroomed.
