I always wear a cup since the day I connected a single shot from my SL-68 II to the 'nads of a buddy as he was diving behind a tree. He jokes about that shot to this day but he wasn't laughing that day!
I wear gloves about half of the time - depends on how hot it is. Once in a great while I break out my neck protector but it rarely gets use. My knees aren't what they should be so I always wear knee pads over my pants.
I always wear long sleeves and long pants (BDUs) while playing. Makes some of those close shots less painful.
Full face mask goes without saying...

Retired from paintball
I wear a Ban Du, long pants (BDUs), half finger ThunderWear gloves (original ones), goggles, back support for scenario games (big & fat, not good for the ol' back!), Under Armour support shorts, and long sleeves if it's cool or cold out otherwise short sleeves unless wearing full BDUs.
The support shorts are great! They really help keep your legs from feeling really wobbly after a long day of playing. I haven't worn a cup in years, but it looks like it's time to start again. I had at LEAST 10 shots between my knees and waist at the last scenario game, with several within a few inches of the Nether Region.
