You do not need to use your real name for the list, you can give a nickname. If I do not receive a name for the list I will use the beginning portion of the email attached to the payment or something similar from the payment information.

No. All that means is every bag is an entry. Each one no matter how you order equals a seperate entry.
Shipping. Shipping is included in the price.
