I'm not much for explaining the rules using the appropriate terminology but I can tell you how to test a sentence.

"I helped my uncle, whose name is Jack, off a horse."

Anything between the commas can be removed, without fundamentally changing the meaning of the sentence.

your uncle's full name is "off a horse jack."
