It does look like it failed where the hole was drilled.
It's unfortunate the pics are over-exposed in places due to reflections from the flash, I'd like to see a more detailed shot of area where the failure occured.

There doesn't seem to be any of the blackening that was visible in the pictures of the tank that went off from the presence of oil.

It looks like it was DEFINITELY over torqued... also, have any of you seen the episode of myth busters where they shear off the valve on a scuba tank and it goes through the cement wall?

wouldnt the TANK be the missle moreso than the reg in this case?

how about pics of the wall damage, and any other damage to the tank? better closeups of the reg and parts as well would be great.

Well, he said the tank went one way, reg went the other (much faster). The point of failure makes plenty of sense, but look at the fracture pattern... something about it just looks weird to me.

Pretty much, I'm not gonna buy cheap regs anytime soon, that's for sure. I think I'll stick to the older Crossfire, and Pure Energy, and any AA or Centerflags I can find. That's some scary stuff.

^ nope. (rebranded thing)

I honestly think, like I said before, that this is a user inflicted thing. DXS has to test their regs to exceed the pressure. They don't use ****ty metal, it's actually a good reg. They are just longer than most. I have a dxs tank as well but like said before, you don't hear about this happening in the US. The dxs reg has both burst disk (the hp and lp one) so once again, I don't think it was "exploded", I think that the dumbass locktited it in the like dropped it. Why do I think this? The reg isn't going to shoot off into the wall, it's the air tank that will. The reg has no real force behind it except for that slight moment when it breaks off. Also the tank was in middle of room (near dinner table .. most of us have those in the middle of a room), so I truly doubt ti had enough force behind the reg to shoot it across the room and into the wall. One more thing, If the reg saftey devices fail at all (burst disks), the gauge would have blown ... and clearly it's not. Just looks either over torqued or dropped to me.

Well, I'm staring at a myth reg right now, and there are no holes. There are channels that would seem to perform the same function, but they would'nt ruin the integrity or strength like a hole would. I mean, that reg snapped right at the hole, that can't be a coincidence.

That's what I've heard too.

And I don't think anything about a 3k or 4.5k charged tank breaking open that's small. Even if the regulator catches a little bit of the force when it's being blown off (which it will) it will certainly be thrown a considerable distance with enough force to dent the top of the reg. Besides, dropping a tank wouldn't cause it to blow up like that. If it did, I'd be full of regulator shrapnel.
