Leaky reg

Hi guys, have the stock reg on my 99' Cocker and it was leaking out the vent hole on the side. Don't know what this could mean as it was lubed nicely and all the O-rings were in the there and in good shape. Could I somehow have put the shim stack in wrong? Thanks.
